Do you need someone to guide you through your health insurance options?
NYS Certified Navigators: We meet with you to provide free information and assistance to applicants without pressure or obligation. Healthy Capital District staff work directly with individuals to complete applications through the NY State of Health Marketplace and apply for Medicaid, Child Health Plus, Essential Plan, and Qualified Health Plans. Insurance subsidies are available for people who make up to 400% above the poverty line.

You can walk into Healthy Capital District at 175 Central Avenue, 5th floor, Albany NY, Monday-Friday, between 9am and 3pm and be seen by the next available navigator.

To meet with a navigator close to your home or work, call for an appointment to be seen at these locations: Ellis Hospital (McClellan campus), St. Mary’s Hospital, and the Cohoes Senior Center.
Are you a Senior, Disabled, or Blind?
Facilitated Enrollment: Insurance coverage for individuals with specialized needs can be more complex to coordinate. Healthy Capital District staff assists in the application process for seniors, disabled and blind individuals for Medicaid, Medicaid Buy-in Programs for working Disabled, Medicaid Spend down Programs, Medicare Savings Program, and help with Medicaid for Nursing Homes. This program is for residents of Albany, Saratoga, Schenectady, and Schoharie County. (518) 462-1459

Insurance for Adults
- Medicaid – free health insurance for low-income adults
- Essential Plan – new low-cost health plan for individuals/New York residents 19-64 years of age through the NY State of Health Marketplace. The premium cost is $0 per month.
- Medicare Savings Program – help with paying your Medicare Part B premium
- Qualified Health Plan – affordable health plans through the NYS Marketplace & Affordable Care Act
Insurance for Children
- Medicaid - free health insurance for low-income families
- Child Health Plus –free or low-cost health insurance for all children that are NY State residents
