The Role of PHIPs
HCDI, as the Capital Region Population Health Improvement Program (PHIP), supports the Prevention Agenda by providing up to date health data, facilitating the prioritization process, supporting the development of community health improvement plans throughout the region, measurement plans to monitor plan efficacy and information on evidence-based practices to positively impact regional priorities.
